es gibt ja die messages WM_[L|R|M]BUTTON[CLICK|DBLCLICK|DOWN|UP]
aber wie registriere ich weitere Mousebuttons?

Alles andere hängt ausschließlich vom Treiber ab.
Evtl. kommt man mit DirectX noch an mehr Infos.

Gibts da irgend ne neuere Version? ich hasse dieses monster msdn library, die is so unübersichtlich und langsam

Ja sie ist groß und es gab schon bessere Iterfaces, aber sie ist durch nichts aktuell zu ersetzen.
