ich habe mehere CLients, die den gleichen COM-Server nutzen.
Wie schaff ich es, dass der COM-Server nur einmal exsistiert und alle Clients auf die gleiche Instanz meines Servers zugreifen?

Darein setzt Du einfach nen Zeiger auf das erste erstellte Objekt und gibst dann bei CreateInstance immer diesen Zeiger zurück. P.S. den RefCount nicht vergessen zu erhöhen.
